Patient Update & Online Registration Form

Our upgraded service is linked to our patient records system and is called ‘VisionOnline’. The system allows patients to request repeat prescriptions with up to date prescription information from the patient record.

Patients can also book and cancel appointments on-line. These are both additional services, and patients will still be able to book appointments by phone and order prescriptions by handing in the repeat re-order slip.

If you wish to use the online-service please use the Update Form to enter your e-mail address to request your secure log-in details. You will need to bring this form to the Health Centre, along with a valid form of photographic identification such as a passport or a driving licence. PLEASE DO THIS BETWEEN 3PM AND 5PM. You will then be given your Registration letter with details of how to create your secure account. Click on the VisionOnline link above to access the system.

Please be aware that if your e-mail address is shared by a number of people in your family, all of them will be able to see details of appointments made or cancelled using the system. If you require more than two forms for your household, they are available from Reception or they can be printed from the link above.
